Nurse’s Office Procedures:

  • The office is open during school hours.
  • Students must report or their classroom teacher for a pass before coming to the health room except for extreme emergencies.

Parents/Guardians of Sophomores:

Please remember to have your student’s physical examination completed BY June 1st.  State Law requires every student in Grade 10 to have a physical prior to this date. The required “blue form” has been mailed home this fall, but should you need a new copy, please obtain a new one from the nurse’s office. Have your student stop by to pick one up or call us and we’ll mail a new one to you. If you have not set up an appointment for a physical to be completed, please do so. At the physical, be sure to have the physician include a hemoglobin or hematocrit with the exam if the physical feels it is needed. Do not hesitate to call us if you have any questions.

Parent/Guardians of Incoming Juniors (2016-2017 School Year):

State law requires that each student enrolled in the public school system have documentation of a completed “health assessment” (a “physical”) PRIOR to entering their junior year of high school.  If you have not set up an appointment with your student’s physician to address this, please do so.  Any student who does not have a physical on record before the start of their junior year will not be allowed to return to school until the documentation is on record.

Please have your child’s physician complete the Health Assessment Record (form HAR-3)which can be found on this website and return it to the nurses’ office.  (Sports forms cannot be accepted for this purpose).
