Entry Requirements
State of Connecticut and Wethersfield Public Schools
Requirements for School Entry
Student registrations for Grades
1-12 of the Wethersfield Public Schools will be conducted at the Board of
Education, Stillman Building located at 127 Hartford Avenue. Documents
that will be required are students long-form birth certificate, and rental
lease agreement or tax assessment and a valid driver's license. Should

Health Requirements:
This
information has been prepared to assist you in meeting the health
requirements for entry to the Wethersfield Public Schools. If you have

* if
entering Kindergarten or a higher grade, the last dose must be after
the fourth birthday.
MMR (Measles,
Mumps, Rubella) must be given after the first birthday. A second measles
dose is required for those in grades K-3 and 7-12.
Hepatitis B - 3 doses for all students in grades K-4, 8-10. One dose
for those entering 7th grade.
Varicella - One dose for students born after 1-1-97 and entering
grades 7-10.
Students may not enter school without first submitting written proof of
all the above immunizations.
Physical
Assessment
The Connecticut Health Assessment Form must have been completed by a
physician no more than one year prior to entry in the Connecticut Public
Schools for all students entering school for the first time, entering from
either private or parochial schools, or entering from out of state.

completed prior to school entry.
Information for Children with Special Needs
Our goal is to enable all Wethersfield students to realize their full
potential in the educational setting. Frequent communication and teamwork
between the student, the family, and the professional staff are necessary to
ensure academic success. Therefore, we request that you make an appointment
with the school nurse in the school your child will attend if your child has
any special health needs (i.e. chronic health problems or allergies)
or if you have any concerns regarding your child's transition to our school.
Children Requiring
Medication
Children who require medication in school must have on file with the
school nurse a written order from their physician and written permission
from a parent or guardian. The parent must supply the medication. These
forms may be obtained from the school nurse.
If you wish to speak with the school nurses, please call 571-8350 and
ask for the school nurse at the school your child will attend.
School Health Services
Each Wethersfield School is staffed by a professional registered nurse
who oversees the school health program.
The school nurse works as a liaison with other school personnel, families,
and the community regarding the health and well being of the students.
The following is a partial list of the services provided on a routine basis.

Impedance Testing

- Nursing assessment
of student illness or injury occurring during school hours.
- Prevention of
disease transmission.
- Administration of
medication and provision of medical treatments, if necessary, during
school hours.

health needs of children
