• Forms Required for Sport Registration:

  • Emergency Athletic Information Form*

  • Physician Consent Form*

  • Pre-Season Medical History Screen*

  • The Student Athletic Handbook is currently not available online.
    Copies must be picked up in the WHS Athletic Office and signed by athlete and parent/guardian.
     

• Travel Release Form*:  Authorizes a student athlete to be transported TO/FROM an away athletic contest by a parent/authorized adult in lieu of bus transportation provided by the Athletic Department. This form must be signed and completed by a parent/guardian then presented to the Athletic Office for authorization and coach notification prior to the dismissal of school on the day of the contest.

Daily Sports Hotline: 571-8289 - A recording of athletic events occurring that day and directions to away games.  Updated throughout the day as changes occur due to weather, etc. 

• Fall 2009-2010 Sport Registration Dates: August 24 – 28

• Winter 2009-2010 Sport Registration Dates: November 16 – 20

• Spring 2010-2011 Sport Registration Dates: March 15 – 19  

NCAA Eligibility Center: For College Bound Student Athletes  (if he/she plans to play sports in college).
